Fig. 3 other:Article Title: Endemic multidrug-resistant Pseudomonas aeruginosa in critically ill patients. Article Snippet: Pseudomonas aeruginosa is a common pathogen among critically ill patients, and strains that have become multidrug resistant are increasingly isolated in both epidemics and endemic colonizations and infections in the intensive care unit (ICU).1-15 This increase suggests the need for better insight into their epidemiology to limit selection, spread, and pathogenicity.. The risk factors for development and routes of acquisition of multidrug-resistant strains of P. aeruginosa in endemic ICU colonizations and infections are largely unknown, although better understanding could have prophylactic or therapeutic implications.. For instance, constraining spread of exogenously acquired multidrugresistant P. aeruginosa via cross-colonization would primarily necessitate improved infection control measures, whereas prevention of colonization and infection by endogenously selected strains would primarily require the elimination of appropriate risk factors, if possible.11,16 There are only a few reports on the risk factors for airway colonization and ventilator-associated pneumonia (VAP), a major cause of nosocomial infection in the ICU, caused by multidrug-resistant bacteria or multidrugresistant or susceptible P. aeruginosa.2,4-6,8,14,17 Risk factors included underlying immunosuppression, a fatal medical condition, coma, emergency intubation, aspiration, chronic obstructive pulmonary disease, prolonged use of certain and multiple antibiotics, a prolonged duration of stay and mechanical ventilation, and invasive procedures, contributing to selecting or acquiring strains endogenously or exogenously, respectively.2,4-6,8,14,17 With the use of genotyping, authors have described that up to 50% of the multidrug-resistant strains arise de novo and the other 50% are transmitted from patient to patient via cross-colonization, even when occurring endemically rather than epidemically.3,6,10 Others, however, described that horizontal transmission via cross-colonization in their ICUs was a relatively rare cause of endemic infections, including VAP with gram-negative bacteria such as P. aeruginosa.4,6,9,10,18 Finally, the consequences of harboring multidrugresistant strains remain unclear. |
